CereCore is seeking a Pharmacist that will work Remotely for our client based in Indiana.
Summary
The COE Pharmacist is responsible for monitoring, evaluating and making recommendations to assure the appropriate and effective use of pharmaceuticals for patient care. The COE pharmacist shall use the medication process to assess, plan, intervene when necessary, implement and evaluate patient care in compliance with client hospital policies, protocols and procedures
Requirements
- Approving orders, processing orders, validating orders, and clinical dosing, managing the vigilance high alerts at night.
- Provides patient care activities to ensure safe and effective drug therapy
- Is of service to patients, physicians, nurses and other pharmacists
- Maintains and improves job knowledge, skills and competence
- Practices and adheres to “Code of Conduct” philosophy and “Mission and Value Statement”
- Performs duties in accordance with specific standards of care as evidenced by completion of competency skills checklist
- Articulates daily activities that support continuous quality improvement of patient care
- Prepares written or oral report at change of shift to communicate incomplete assignments and potential patient safety concerns to monitor in order to ensure continuity of medication therapy
- Demonstrates knowledge and mastery of required job functions, internal communications and operating systems
- Accurately enters orders in the computer in a timely manner
- Supports and assists in meeting departmental goals for order entry and clinical programs
- Ensures facility procedures are followed when non-formulary drugs, target drugs, restricted drugs and investigational drugs are ordered
- Initiates change in assignments based on workload and resources available
- Demonstrates proficiency in computer order entry and related computer functions
- Screens for drug interactions, allergies, order duplications, appropriate diagnosis, renal and liver function prior to order entry
- Assures facility queues are addressed in a timely manner
- Investigates and reports adverse drug events and medication incidents
- Facilitates dissemination of drug information in a timely manner to achieve high quality, cost effective, positive therapeutic outcomes when consulted
- Reviews and interprets all physician orders received, using the patient profile
- Monitors for incompatibilities, concentration and rate on intravenous drugs
- Participates in the maintenance of continuous quality improvement program
- Assists in the development and review of COE protocols
- Documents clinical interventions and follow-up when indicated
- Assesses orders for age-specific appropriateness from neonatal through geriatric
- Follows CSC pharmacy policies and procedures
Requirements
- Indiana Pharmacist License Required
- Bachelor’s degree from an accredited college of pharmacy. Pharm D preferred.
- One (1) year hospital experience is preferred
- Experience working with MEDITECH preferred
- Experience with Vigilance is preferred
