Position Summary

The Hospital Case Management Implementation Coordinator (IC) will ensure the hospital case management technology is effectively integrated into hospital operations, enhancing patient care, streamlining workflows, and improving overall efficiency. This role will regularly engage with clinical staff as a subject matter expert to ensure they have appropriate access and proficient in using the software system's functionality. This role is a workflow and system expert who will support the facility/market during the implementation and go-live periods. This position requires a detailed understanding of the system functionality and familiarity with facility case management clinical workflow. This team member will be responsible for ensuring completion of pre-go live tasks, coordinating implementation initiatives at the facility, and supporting training and troubleshooting pre and post go-live. The implementation coordinator will be called upon to support other planning and implementation tasks as needed to support the overall program goals. This role should expect to travel to multiple HCA facilities, with the majority of their time spent in patient care units.


Responsibilities

  • Apply knowledge of software and workflow during go-live and implementation independently and without direct supervision.
  • Provide support to Case Management Staff, Preceptors, and the Case Management Director, ensuring they have appropriate access and can demonstrate efficient use of the software.
  • Perform initial troubleshooting during go-live and ensure system stability, serving as the point of contact for all associated software issues.
  • Promptly escalate any access or software issues to avoid delays in colleagues' ability to access and work in the system
  • Ensure a standardized and consistent approach relating to education delivery.
  • Participate in communication among all team members to ensure a comprehensive understanding of needs, expectations, and requirements.
  • Evaluate the effectiveness of the implementation strategy and propose modifications of materials as needed.
  • Participate in initial kickoff and work in close coordination with corporate, division, and facility leadership.
  • Assess the lessons learned from each phase of implementation and integrate these insights into subsequent phases.
  • Organize and support onboarding of new resources and team members during the implementation period.
  • Coordinate with the Implementation team to ensure all facility case management staff and shifts have necessary go-live support.
  • Perform other duties as assigned

Requirements

  • Experience with CarePort Care Management
  • Ability to Establish and maintain collaborative and effective working relationships.
